Conduct surveys and schedule reassessments

Once a survey has been built (see  Build and manage surveys ), staff conduct it from a client's record — recording the client's answers on their behalf — and can schedule recurring reassessments. This guide covers taking surveys and scheduling them for clients.

Open a client's Surveys section

Navigate to a client's record and open the Surveys section. It has three areas:
  • Responses — surveys already recorded for this client.
  • Scheduled Surveys — surveys due to be completed.
  • Start a new survey — surveys available to conduct now.

Start a survey

    Under Start a new survey, find the survey and click Start Survey .
    Enter the client's answers. A progress bar at the top shows how many questions have been answered ("X of N answered").
    Click Submit to record a completed response, or Save Draft to save your progress and finish later.

View, resume, and edit responses

Completed and draft responses appear under Responses:
  • A Draft badge marks an unfinished response. Click Resume to continue and submit it.
  • Click View Response to open a completed response, then Edit Response to make changes.
  • When the survey uses scoring, a Score badge shows the response's total.

Schedule a survey or reassessment

Schedule a survey so it appears as a reminder to complete later — useful for periodic reassessments.
    Under Start a new survey , click Schedule next to the survey.
    Choose a Due date .
    Choose a Recurrence : One-time , Monthly , Quarterly , Every 6 months , or Annual .
    Click Schedule Survey .
Scheduled surveys appear under Scheduled Surveys with their due date; overdue items are highlighted in red. Click Complete to take the survey, or Cancel to remove the schedule. Completing a scheduled survey marks it done and, for recurring schedules, automatically creates the next occurrence.
Reassessment reminders. Staff receive a daily reminder of due and overdue reassessments. You can review them all on the Survey Assignments report — see  Survey reports .
